Behavioral Health Social Worker

The Trempealeau County Human Services Department is seeking a qualified individual to join the Behavioral Health Services Team to coordinate and facilitate services for consumers enrolled in the Community Services Program (CSP) and the Comprehensive Community Services Program (CCS). This position assists CSP and CCS consumers to maintain stability in the community by developing and monitoring service plans, providing case management services, assessing risk/protective services, coordinating psycho-social rehabilitation services and providing consumer advocacy in a person centered approach.   The person in the position will have a demanding caseload and work in a fast-paced department. The work is performed by a Social Worker in accordance with applicable rules, regulations, guidelines, and laws for the Comprehensive Community Services (CCS) program and under the direction of the Behavioral Health Services Supervisor.
