Social Worker - ADRC Outreach and Disability Benefits Specialist

The purpose of this position is to provide two unique types of services within the Aging and Disabilities Resource Center (ADRC) to assist individuals with disabilities and the aging population. A portion of the position is to provide disability benefit information and assistance, options counseling, and resource development services to individuals ages eighteen through fifty-nine years old with physical and/or developmental disabilities. The other portion is to provide information and assistance, resource development, community development, marketing and public outreach for the ADRC Services. This position would work with community partners to help connect the community to the ADRC Services and Volunteer Services. The work is performed under the direction of the ADRC/Senior Services Supervisor.

Public Health Technician

The purpose of this position is to assist with daily operation of the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) and the Health Department. This includes office operations and caseload management of the WIC Program, obtaining anthropometric data and lab samples, office operations of the Health Department, and providing support to residents of Trempealeau County for all Health Department programs, including interpreting for Spanish speaking residents. The person in this position will fill-in on an as needed basis for any of the following routes: Osseo, Eleva / Strum, Whitehall / Pigeon Falls, Independence, Arcadia, Blair / Ettrick / Galesville and rural Galesville / Trempealeau working Monday through Friday between the hours of 10:00 a.m. - 1:00 p.m. up to 10 hours per week. Flexible scheduling available. The work is performed under the supervision of the ADRC/Senior Services Supervisor.

Social Worker - Dementia Care Specialist

The purpose of this position is to provide Trempealeau, Buffalo and Pepin Counties with the knowledge and resources regarding older adults, dementia, dementia care and support, family and informal caregiving needs as well as challenging behaviors. This position is unique in the aspect that it will serve all three counties although Trempealeau County will be the primary office and employer. The Dementia Care Specialist Social Worker will also provide community development, marketing and public outreach for the Aging and Disability Resource Center (ADRC) Services. This position would work with community partners to help connect the community to the ADRC Services and local volunteer services. This will include working closely with community organizations and health care professionals to build Dementia Friendly Communities. This position will provide volunteer training and guidance to ensure there is access to the ongoing resources for people living in our communities. Also, providing outreach and awareness to the community about the ADRC and available Dementia Services as well as providing short-term service coordination for individuals with dementia and/or their caregivers. The work is performed under the direction of the Trempealeau County ADRC/Senior Services Supervisor.
